Omen of Amelioration did not trigger on death.

I died in a mirage on a T16 map (Mesa) and the Omen is still in my inventory and the XP loss is the full 10%, which is painful on lvl 99.

Did you have a different omen in your inventory that triggered first? Only one omen of any kind can trigger per map.
